 I hereby release and hold harmless Carol Gurney  and/or their agents, from liability due to accident or damage to self or animal, and/or personal property, and from loss of property during any workshop or consultation.

 I as owner, or agent of the owner, agree to have this consultation for animal(s) and give my consent by completing payment or acting in a way to complete payment for any consultation .  This denotes acceptance of the agreement and release form.
